Your name, surname, Turkish Republic ID number, your passport number or temporary Turkish Republic ID number, birth place and date, marital status, gender information and identity card or driver’s license photocopy you submit, if you are not a Turkish citizen,

 Transferring Personal Data

Your personal data, the Basic Law of Health Services No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Organizations, the Law on Protection of Personal Data No. 6698, the Regulation on Private Hospitals, the Regulation on the Protection and Privacy of Personal Health Data and the Ministry of Health regulations and other regulations. within the framework of the provisions of the legislation and for the purposes described above;
